What’s the program about?
Parents, carers or guardians can apply for a voucher valued up to $150 for their child or young person, which can be used towards membership, registration or participation fees with registered activity providers.The Tableland Futsal Club is a registered activity provider.
Who’s eligible?
Eligible applicants are Queensland children and young people aged 5 to 17 years (inclusive) who either:
- hold or whose parent/carer/guardian holds a valid Centrelink Health Care Card or Pensioner Concession Card with the child’s name on it.
- are identified by a registered referral agent.
Please note that there is a limit of one voucher per child/young person per calendar year, applications are open twice a year. If a child receives a voucher in the summer (January-April) round of the program, they will not be eligible to apply for the winter (July-September) round of the same year.
The expiry date for Round 4 vouchers is 12 May 2021 and the voucher redemption expiry is
26 May 2021.
